Customer Survey Drawing — Official Rules

NO PURCHASE OR SURVEY RESPONSE NECESSARY TO ENTER OR WIN. A PURCHASE OR SURVEY RESPONSE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED.

1. Sponsor. Doingwell, Inc., 8118 Beverly Blvd., Los Angeles, CA 90048 ("Sponsor").

2. Eligibility. Open to legal residents of the 50 United States and D.C. who are 18 or older at time of entry. Employees, contractors, and brand creators of Sponsor, and members of their immediate families and households, are not eligible.

3. Entry period. Begins 12:00 a.m. PT on September 28, 2026 and ends 11:59 p.m. PT on October 12, 2026 ("Entry Period").

4. How to enter. (a) Complete a doingwell customer survey (hydration or POG) sent by email during the Entry Period; or (b) Free method: email team@doingwell.health with the subject "Survey Drawing Entry" and your full name and mailing address. Limit one (1) entry per person regardless of method or number of surveys completed. Entries by either method have an equal chance of winning.

5. Prize. One (1) $100 doingwell store gift card, redeemable at doingwell.health. Approximate retail value: $100. Not redeemable for cash; no substitution or transfer except by Sponsor, who may substitute a prize of equal or greater value.

6. Drawing. One winner will be selected at random from all eligible entries on or about October 13, 2026. Odds of winning depend on the number of eligible entries received.

7. Winner notification. The winner will be notified by email within 3 days of the drawing and must respond within 7 days, or an alternate winner will be selected.

8. General. Entries that are incomplete, automated, or submitted through fraudulent means are void. Survey answers have no effect on eligibility or selection. Sponsor is not responsible for lost, late, or misdirected entries or technical failures. By entering, participants agree to these rules and Sponsor's decisions, which are final.

9. Privacy. Information collected is used to administer this drawing and is handled per Sponsor's privacy policy at doingwell.health/policies/privacy-policy.

10. Winner's name. For the winner's first name and state, email team@doingwell.health with the subject "Survey Drawing Winner" by November 15, 2026.
